For a Public Records Request Form for non-medical and non-injury reports/incidents:

 Any medical or incident report requests for official fire department records must be submitted in writing through our application with a copy of your ID and payment. Each request will cost minimum $15.00 and can be paid with either cash, check or credit card (from the main menu select I WANT TO-> Make a Payment). Your request will not be processed until we have received payment and the appropriate documents.
